NOTE: Effective January 1, 2021, the DPSA authorized the revised Z83 Application Form (available from any Public Service department); applicants are encouraged to utilize the new form, since failing to do so would result in their application being dismissed.

When submitting the Z83 form, it must be signed. To expedite the recruitment process and make it more responsive to the public, as well as to provide more protective measures during the pandemic by minimizing overcrowding and lowering candidate fees, the following certification procedures should be implemented: To represent that application, the advertisement and related notes must explicitly capture the certification criteria. Please keep in mind that although candidates are not needed to provide copies of their credentials or other necessary papers with their application, they must submit the Z83 and a complete curriculum vitae.

Only selected applicants will get information from the department’s HR about the need for verified papers. Candidates who are chosen for interviews will be asked to supply extra certified information that may be sought to make a final choice, according to the subsection “additional information” on the application for employment Form (Z83).

It’s important to remember that when a document is certified as a genuine copy of an original, the certifier is merely confirming that it’s a true copy of the original.
